Formify is a Discord bot and web dashboard for building forms, reviewing submissions, and running support tickets inside your server. This is the official documentation for the Formify Discord bot at formify.pro.

Most people set it up like this:

  1. Add Formify to your server
  2. Open the dashboard and pick your server
  3. Create a form and publish it to a channel
  4. Review responses in Discord or under Submission Logs

You can also create basic forms with /create in Discord, but the dashboard gives you more control over questions, embeds, roles, and tickets.

Forms vs tickets (short version)

  • A form collects answers from members. Submissions land in a review channel (or thread/forum, depending on your settings).
  • A ticket template opens a private support channel when someone clicks a button or picks an option from a panel.
  • Ticket logs in the dashboard are the list of support channels that were opened. Tickets in the sidebar are the templates you publish — not the open channels themselves.
