Submissions
When someone completes a form, Formify creates a submission. Staff review it in Discord (response buttons) and/or in the dashboard under Submission Logs.
Where submissions appear
Depends on your form's Output settings:
The dashboard Submission Logs always lists submissions for your server regardless of output type.
Review in Discord
The review message includes buttons such as:
- Approve / Deny / Ignore
- Ticket Response — open a support ticket tied to the submission
- DM Response — send a custom message to the submitter
- Poll It! — post a poll in a configured channel (if enabled on the form)
Who can press these buttons:
- Users with a mod role on that form, or
- Anyone with Manage Server
Each action is logged on the output message as an activity trail, so the next moderator sees what already happened.
Review in the dashboard
- Go to Submission Logs.
- Browse, search, or sort:
- Search by submission ID, user ID, or form ID
- Sort newest or oldest
- Open a submission to see:
- All answers
- Activity history (approvals, DMs, tickets, etc.)
- Linked ticket info, if any
You can delete one submission or select several and bulk delete.
Note: Approve/Deny/Ticket actions happen in Discord, not from the submission detail page. The dashboard is for reading history and cleaning up records.
Export to CSV
To download every response for one form:
- Go to Forms.
- Open Manage on the form.
- Click Export CSV.
You can also export from the form editor's manage menu. The file includes all stored answers for that form.
There is no CSV export for ticket logs or audit logs.
Reset a user's submission
If Only submit once is enabled and someone needs to re-apply, a moderator with Manage Server can run:
/resetuser form:<id> user:<member>
Clear old submissions
/clearsubmissions form:<id>
This deletes submission records from the database. Old Discord messages may still exist, but response buttons on those messages will no longer work reliably. Use this only when you intentionally want a clean slate.
